Quality Measures Rating Methodology
Which measures are included? - The Quality of Patient Care Star Rating methodology includes 13 of the 24 currently reported process and outcome quality measures. These measures were chosen based on the following criteria:
- The measure should apply to a substantial proportion of home health patients and have sufficient data to report for a majority of home health agencies.
- The measure should show a reasonable amount of variation among home health agencies and it should be possible for a home health agency to show improvement in performance.
- The measure should have high face validity and clinical relevance.
- The measure should be stable and not show substantial random variation over time.
Outcome measures: - Improvement in Ambulation
- Improvement in Bed Transferring
- Improvement in Bathing
- Improvement in Pain Interfering With Activity
- Improvement in Shortness of Breath
- Acute Care Hospitalization

How to Run the Report
o From / Thru = Based on dates using sequence date on the OASIS.
o Program(s) / Payer(s) = Filter report by entering program, payer(s). Or leave blank to select all.
o Active Patients Only = Will select patients who are currently active.
o Discharged Patients Only = Will select patients who are currently discharged.
o Skip Those With Only 1 Answer = Skips patients who have only one answer within the selected OASIS question.
o Only Show First and Last Answers = When patient has more than two OASIS containing answers, only the first and last will appear.
o All = Selects all OASIS regardless of answer.
o Got Worse Only = Only selects OASIS where the answers got worse.
o Improved Only = Only selects OASIS where the answers improved.
o Mixed Answers = Select OASIS that did not stay the same, got worse or improve.
o Click on Print.
How to Read the Report
Depending on the report options you picked to run, the report will show the patient's name and the answers to the start quality rating OASIS questions. There is a legend at the top of the report to help read the answers. When multiple answers appear on the report, they will be separated with (+) = improved, (-) = declined, (.) = stayed the same.
OASIS questions that appear on the report include:
- M1041 = Influenza Vaccine Data Collection between 10/1 - 3/31
- M1046 = Influenza Vaccine Received
- M1051 = Has patient ever received the pneumococcal vaccination
- M1056 = Reason Pneumococcal Vaccine not received
- M2015 = Patient/Caregiver Drug Education Intervention
- M2420 = (DC only) Where is the patient after discharge from your agency
- M1242 = Frequency of Pain Interfering
- M1400 = Shortness of Breath
- M1830 = Bathing
- M1850 = Bed Transferring
- M1860 = Ambulation/Locomotion
