What must be in the EVV electronic record?Answer: The EVV electronic record must contain the six data elements required by the CURESAct. This includes (i) the type of service performed; (ii) the individual receiving the service; (iii)the date of the service; (iv) the beginning and ending location of service delivery; (v) theindividual providing the service, and; (vi) the time the service begins and ends. For Home HealthCare Services only one location is required.
